What is your contact information? Who is your best point of contact for our nurses and social workers? When are the best times to be reached? Can your facility accommodate virtual visits?

Are you a secure facility? Does your facility accept clients at risk for wandering?

Does your facility offer private rooms?

Does your facility accept clients that require regular injections, i.e. sliding scale insulin?

How many open beds does your facility currently have? Is your facility an RCFE or an ARF?

Does your facility cater to clients with pets? If so, are there any restrictions?

What languages is your staff proficient in, other than English?
