With over 1,000 home health clients served and more than 3,500 clients assisted in applying for the Assisted Living Waiver Program (ALWP), our team is dedicated to providing compassionate, professional care across Southern California. The Assisted Living Waiver Program (ALWP) helps eligible individuals transition into assisted living facilities from institutional or community settings. As a Department of Healthcare Services licensed care coordination agency for the waiver program, Media Home Health assists clients who have Medi-Cal with navigating the application process and ensures that they receive the services they need to thrive in a community-based setting through monthly progress visits and nurse evaluations.
