Media Home Health strives to provide peace of mind combined with exceptional patient-centered care and consistency for you or your loved ones. Our highly skilled nurses, social workers, and therapists pride themselves on their holistic and innovative approaches to wound care, chronic disease management, and rehabilitation. At Media, we take communication and availability seriously and are always just a phone call or email away. 


We offer a range of home health care services including skilled nursing, physical therapy, occupational therapy, speech therapy, and more.


We also offer care and coordination for our Assisted Living Waiver Program (ALWP) and our CalAIM ALF Transitions Program.


We accept Medicare, Medi-Cal, and most private insurance plans for home health services.       Medi-Cal is a requirement for the Assisted Living Waiver Program.


To begin home health, ALWP, or CalAIM Program services with us, you can call us at (818) 740-3767 and our helpful staff can assist you with applying. ALWP and CalAIM applicants can also fill out the inquiry forms on our program pages.

Medicare covers 100% of home health costs if the patient meets eligibility. Private insurance or Medi-Cal may have copays or limits.


Someone seeking home health services must require skilled nursing care or therapy services while being homebound. 


Speak with your primary care doctor, specialist, or hospital discharge planner about your need for home health care. Your doctor must determine that you require skilled nursing or therapy services and order home health care for you. Our team can help you every step of the way!


We service the greater Los Angeles area and most of Los Angeles county.


Whether you or your loved one resides at home or in an assisted living community, we are happy to provide our services in any residence!


 Home health care is medical and provided by nurses/therapists. Home care (personal care) is non-medical and includes assistance with bathing, dressing, meal prep, and companionship. 


Our team members take on every case with a personalized level of care and attention and will follow you or your loved one throughout the duration of your care.


Assisted Living Waiver Program (ALWP)

  • Reside in an assisted living facility at a reduced rate instead of a nursing home or institutional setting
  • Receive assistance with ADLs including but not limited to personal hygiene, toileting, dressing, and bathing
  • Receive housekeeping and laundry services
  • Receive assistance with medication management


  • Application assistance and processing
  • Care coordination and placement
  • Monthly progress visits with a licensed social worker
  • Biannual nursing evaluations with a licensed registered nurse (RN)
  • Assistance with Medi-Cal eligibility and share-of-cost removal


  • Be age 21 or older
  • Have Medi-Cal coverage
  • Require assistance with activities of daily living (ADLs) including, but not limited to, assistance with personal hygiene, toileting, dressing, and/or bathing
  • Meet the SSI/SSD eligibility criteria
  • Willing to live in an assisted living facility located within the following 5 Southern California counties: Los Angeles, Ventura, San Bernardino, Orange, and San Diego
  • If applying from an institutional setting, you must reside there for 60 consecutive days before applying


  • Stage 3 or 4 pressure sores (pressure ulcers) or other severe unstageable ulcers or deep tissue injuries
  • Nasogastric tubes
  • Ventilator dependency
  • BiPap dependency without the ability to self-administer at all times
  • Coma
  • Continuous IV/TPN therapy
  • Wound Vac Therapy
  • Active communicable tuberculosis (TB)
  • Restraints, except as permitted by the licensing agency for RCFE residents
  • Clients who require transfer or mobility assistance from more than one person in the event of an emergency requiring evacuation


  • Assistance with all personal care needs and ADLs including personal hygiene, toileting, dressing, and bathing
  • Nursing care and support to manage chronic medical and mental health conditions
  • Medication management
  • Monthly care coordination visits led by a team of social workers and registered nurses that will advocate for you and ensure your needs continue to be met while residing in the assisted living facility


  • Fill out the inquiry form below, contact us directly, or speak to your social worker and let them know about us!
  • If in an institutional setting (i.e. nursing home), you must reside there for 60 consecutive days first
  • If residing at home or in a community setting (i.e. an assisted living facility), you will need to be added to our waitlist. The quickest way to enroll in the ALWP is while residing in a nursing home or other long-term institutional setting.

CalAIM Assisted Living Facility (ALF) Transitions Program

Applicants residing in a nursing facility who:

  1. Have resided 60+ days in a nursing facility and
  2. Are willing to live in an assisted living setting as an alternative to a nursing facility; and
  3. Are able to reside safely in an assisted living facility (ALF).


Applicants residing in the community who:

  1. Are interested in remaining in the community; and
  2. Are willing and able to reside safely in an ALF; and
  3. Meet the minimum criteria to receive nursing facility level of care (LOC) services and, in lieu of going into a facility, choose to remain in the community and continue to receive medically necessary nursing facility LOC services at an ALF.


Eligible applicants can usually be assessed and approved within 1-2 weeks of inquiring. Applicants must also agree to be placed on the Assisted Living Waiver Program (ALWP) waitlist, if they have not already done so. Once approved, the CalAIM Program will assist with transition to an assisted living setting and help cover expenses (not including room and board) while the applicant remains on the ALWP waitlist.
