CalAIM Assisted Living Facility Transitions Program

We are now excited to announce that Media Home Health is servicing Orange County for the CalAIM Program for individuals with CalOptima Medi-Cal coverage!


The CalAIM Assisted Living Facility (ALF) Transitions Program serves to bridge the gap between clients currently waitlisted for the Assisted Living Waiver Program (ALWP) who need assistance with transitioning into assisted living and board and care settings. Current CalOptima recipients can apply for coverage through our agency to receive CalAIM benefits, which can help cover the cost of these facilities.


CalAIM coverage through Media Home Health is currently available in Orange County. Stay tuned for coverage expansion to other counties!

Who is eligible for the CalAIM ALF Program?

Applicants residing in a nursing facility who:

  1. Have resided 60+ days in a nursing facility and
  2. Are willing to live in an assisted living setting as an alternative to a nursing facility; and
  3. Are able to reside safely in an assisted living facility (ALF).


Applicants residing in the community who:

  1. Are interested in remaining in the community; and
  2. Are willing and able to reside safely in an ALF; and
  3. Meet the minimum criteria to receive nursing facility level of care (LOC) services and, in lieu of going into a facility, choose to remain in the community and continue to receive medically necessary nursing facility LOC services at an ALF.

What is the timeframe for joining the CalAIM ALF Program?

Eligible applicants can usually be assessed and approved within 1-2 weeks of inquiring. Applicants must also agree to be placed on the Assisted Living Waiver Program (ALWP) waitlist, if they have not already done so. Once approved, the CalAIM Program will assist with transition to an assisted living setting and help cover expenses (not including room and board) while the applicant remains on the ALWP waitlist.
